Clerkship
Step 1: Requesting Rotation
All requests must be submitted at least 60 days prior to the begin date of the rotation. All requests must be faxed or e-mailed to the Clerkship Coordinator.
To request a rotation you need to contact the Clerkship Coordinator at 210-292-7441. The coordinator will let you know if slots are available for the date and program you are requesting.
Once you have been informed that slots are available you will be required to go to the AFIT or USUHS website and asked to download your Active Duty Tour (ADT) or USUHS Form 1304, complete it and fax (210-292-7008) or e-mail it to the Clerkship Coordinator.
Step 2: Upon Acceptance
You will receive a letter from the Director of Graduate Medical Education with instructions regarding lodging, location of our office and reporting time. You will also receive an informational packet, information on the department in which you will be training and a questionnaire requesting information for computer training.
Step 3: Reporting to WHMC
You are required to report to the Clerkship Coordinator’s office NLT 0700. Please bring with you a copy of your orders and immunizations records.
If you decide to cancel a rotation, you must contact the Clerkship Coordinator who will provide you with instruction on cancelling your rotation.
