The PEAK Program is a subsidiary program of the Pulmonary Disease Medicine Clinic, Family Medicine Services, Internal Medicine Clinic, Pediatric Clinic, Allergy & Immunology Clinic, and the Pulmonary Clinic at BAMC. It is a comprehensive asthma self management education program for all BAMC beneficiaries diagnosed with asthma. The program uses a multi-disciplinary approach and offers formal and informal asthma education, development of Asthma Action Plans, routine educational follow up, and increased access to care. Our goal is to have you Reaching Your Peak!
Your asthma team includes you, your family and one or all of the following members:
- Primary Care Provider (Family or Internal Medicine, Pediatrics, Adolescence)
- Asthma Nurse Educator
- Respiratory Therapist
The first step in the PEAK Program is participation in the asthma educational sessions. These are sponsored by the Pulmonary Clinic. Classes are offered every Wednesday from 1300-1430. The classes are held in the Department of Medicine Conference Room on the third floor of BAMC (Room #344-3).
Parent and Adult Class: For 12 year olds and older, parents, caregivers, and all adults
Once you have attended an educational session, you will follow up with the Asthma Nurse Educator or Primary Care Provider to establish your Asthma Action Plan and review key points to asthma control and self management.
After the initial follow up, you will then have periodic follow over one year.
