S4 - In/Out Processing

In Processing

All Soldiers must bring two copies of assignment orders, one assigning them to BAMC WTB, and one to their perspective Company. Soldiers will be in processed under the following categories:

Outpatient Soliders are medically evacuated OIF/OEF soldiers released from the hospital that have been attached or assigned to the WTB.

Outpatient Warriors are medically evacuated OIF/OEF soldiers released from the hospital that have been attached or assigned to the WTB.

  1. If the soldier will be residing in the barracks, on-post housing, or government leased quarters to include guest housing he/she must complete DA Form 2062 (Personal Hand Receipt) to receive a lock set, a laptop computer, a 32" flat screen TV with a built in DVD player, and a cell phone from the WTB S-4.
  2. Complete DA Form 3078 (to receive gratuitous issue, if not previously issued ) with the proper sizes, dates, and signatures.
  3. Complete DA Form 3645-1 (for additional organizational clothing / OCIE) if the soldier receives a safety belt and a US flag patch for their gratuitous issued uniform from the S-4.
  4. Complete DA Form 2823 (Sworn Statement) to start a FLIPL. This form must be submitted for warriors that will not be returning to their home duty stations, this also includes Army Reserve and National Guard soldiers.

Non-combat injured soldier that are non-OIF/OEF and assigned to the WTB and are being treated for an illness or injury due to regular unit related training, or other incidents. These warriors must complete the following forms:

  1. ADA Form 2062 (Personal Hand Receipt) if the soldier resides in the barracks and receives locks, or safety belt from the S-4.
  2. ADA Form 3645-1 (for additional organizational clothing / OCIE) if the soldier receives a US flag patch from the S-4.

Active duty NG/AR E-4 & below soldiers assigned to the WTB in support of the wounded warriors as permanent party will provide the S-4 with a copy of their signed automated CIF list (may be printed from "my clothing" of their AKO webpage). These Warriors must also complete the following forms:

  1. ADA Form 3078 (Initial Issue Inventory) the inventory should be conducted by the squad leader.
  2. ADA Form 3645-1 (Additional Organizational Clothing and Individual Equipment Record) will be signed by the soldier when receiving patches, unit crests, unit citations or reflective belts from the WTB S-4.

AD/NG/AR E-5 & above soldiers assigned to the WTB in support of the wounded warriors as permanent party will provide the S-4 with a copy of their signed automated CIF list (may be printed from "my clothing" of their AKO webpage). These Warriors must also complete the following forms:

  1. An annual Personal Clothing Statement must be completed and signed by the Non-Commissioned Officer.
  2. ADA Form 3645-1 (Additional Organizational Clothing and Individual Equipment Record) will be signed by the soldier when receiving patches, unit crests, unit citations or reflective belts from the WTB S-4.

AD/NG/AR Officers assigned to the WTB in support of the wounded warriors as permanent party will ensure the S-4 is provided a copy of their orders for file maintenance and inspection purposes.

Out Processing

All Soldiers must bring one copy of their orders. Soldiers will be out processed under the following categories:

  1. Outpatient Warriors and Non-Combat Warriors Soldiers are out processed under the same standard. Prior to being cleared by the S4 the following must be accomplished:
    1. A FLIPL (Report of Survey) must be completed and approved for all OIF/OEF soldiers that will be leaving the military and have an outstanding CIF clothing record. Army Reserve and National Guard Soldiers must also have a completed and approved FLIPL.
    2. All soldiers must clear the Fort Sam Houston Central Issue Facility (CIF) before being cleared by the WTB S-4. All soldiers must have a signature and the raised seal stamped on their clearing paperwork from CIF.
    3. Soldiers must turn-in pillows, locks, cold weather issue, laptop computers, 32" flat screen TV's with a built in DVD players, and cell phones to the WTB S-4. Soldiers will not be allowed to clear until they have done so. Upon clearing the S-4 the soldier will receive original copies of his or her hand receipts for all signed out items.
  2. Staff / Permanent Party
    1. Clear the Fort Sam Houston Central Issue Facility (CIF) before being clearing the S4.
    2. The S-4 will provide the soldier with the original copies of his / her clothing file for their next duty station.
